Diabetes

If your child has a diagnosis of Diabetes, please contact your school nurse to discuss the supports that are available at school. 

 

Also, you can click on the FORMS tab to download the Healthcare Plan for Students with Diabetes form.  Have your child's primary healthcare provider complete the form and return it to school, along with any necessary medications and supplies, so that we can fully support your child at school.
